Accessibility review of output of authoring tool

The Preview mode of the Authoring Tool is working now, so I've added review comments about the Preview to the wiki page:

Many of the points are just things I noted, not necessarily accessibility issues. We should probably review the points to clarify whether or not they are problems to be addressed.
