FLUID-1977, FLUID-1978, FLUID-1979: Fixed bug where flash Browse files button wasn't being disabled during the upload process. Made the flash movie always visible by default to work around bug where SWFUpload was freaking out when the Flash movie was transparent.

Minor tweaks to code based on review.
